Just got back from my medical appointment and everything went fine, there's just a form to download and fill in and get witnessed then given to my GP surgery and my next of kin and let all those I see most frequently know my wishes, take a copy of the form and letter to any hospital appointments with me then that's it all done and dusted!
Basically, more official paperwork (like with the body donation to medical science thing) and putting my wishes in writing so that everyone knows about that too then that's it all done and dusted!
I think I've earnt my second hot chocolate of the day lol
